Countdown to ISO 9001:2015 | Quality Digest
The ISO 9001 standard is currently under revision. The decision to do so wasn’t driven by one dramatic event happening in the marketplace, but rather by due diligence of the International Organization for Standardization (ISO) committees and working groups that perform systematic reviews of standards. From a review completed in 2012 plus activities including open workshops with users of the standard, a web-based survey of users and potential users, and studying trends in quality management, it was determined that the standard needed to be updated.....
Historical significance
The organization today known as ISO began in 1926 as the International Federation of the National Standardizing Associations (ISA). It was suspended in 1942 during World War II. After the war, as the United States figured out how to best rebuild infrastructure, other countries were doing the same. ISA was approached by the recently formed United Nations Standards Coordinating Committee (UNSCC) with a proposal to form a new global standards body. So the key stakeholders from ISA and UNSCC representing 25 countries gathered together in October 1946 in London and agreed to join forces to create the new International Organization for Standardization; the new organization officially began operations in February 1947. Since then ISO has overseen the development of more than 19,500 international standards. Based on ISO’s neutrality, it was and still is headquartered in Switzerland.
ISO 9001, the first quality management standard, was originally published in 1987. It was based on the BS 5750 series of standards from British Standards Institution that were proposed to ISO in 1979. However, its history can be traced back some 20 years before that, to the publication of the United States Department of Defense MIL-Q-9858 standard in 1959. MIL-Q-9858 was revised into the NATO AQAP series of standards in 1969, which in turn were revised into the BS 5179 series of guidance standards published in 1974, and finally revised into the BS 5750 series of requirements standards in 1979 ..

GMP News: Frequent GMP Question: How many Signatures are required for Validation Protocols/ Reports?
As part of the validation and qualification activities for the production of medicinal products, the following question often arises: how many signatures have to be put under the respective protocols and reports? What is the responsibility of the Head of Production and the QA department in this matter?
Indications can be found in the chapters 1, 2 and 4 of the EU GMP Guide Part I. You can find in Chapter 4 requirements on the topic "Documentation":
4.2. "Documents should be designed, prepared, reviewed, and distributed with care."
4.3. "Documents containing instructions should be approved, signed and dated by appropriate and authorised persons."

This can be summarized as follows: each document should have one author, one reviewer and one approver. The Head of Production is responsible for the qualification of his equipment and the validation of his processes (Chapter 2.7 (4)); the same applies to the Head of QC in the lab (2.8.).
Since the introduction of the revised Chapter 2 in February this year, a new point has been added. Moreover - depending on the size and the organisational structure of the company - an independent Head of Quality Assurance or Quality Unit can be assigned. Where such a function exists usually some of the responsibilities described in 2.6, 2.7 and 2.8 are shared with the Head of Quality Control and Head of Production. Senior management should therefore take care that roles, responsibilities, and authorities are defined. This means for example that the QA can assume the tasks of the Head of Production. Basically, the QM system should ensure that validation is performed (1.4 (10)).
Conclusion: Depending on your internal structure, you can specify the signature rules. You should be aware of what "approved" and "released" mean as both can have various meanings. "Approved" can be used for a document, for a technical review, for execution, or can even have a completely different meaning in conjunction with other terms. Similarly, depending on the term you use in combination with "released" its meaning is different, as, i.e., released formally or textually. Some companies set such definitions in a SOP documentation.
Finally, the GMP Guide gives the freedom to do it otherwise; if you - at least - come to the same conclusion and can prove it.
In the US, the procedure is slightly different. There, the personal responsibility of the Head of Production / QC is not part of the GMP regulation. On the contrary, it is the QCU who has a crucial role.

The American Society of Testing and Materials (ASTM) has a quite different approach. The document ASTM E 2500 defines "Subject Matter Experts" for qualification. According to this model, the QA department would only sign superordinate qualification documents. This is an interesting and lean approach although not very common Europe yet.

Anvisa aprova mais dois genéricos inéditos para o mercado

Dois novos genéricos, ainda inéditos no mercado, ganharam registro da Anvisa e vão dar mais alternativas para médicos e pacientes. Os produtos são a Nitazoxanida e o Cloridrato de Mebeverina. Os dois medicamentos são de substâncias que ainda não tinham concorrentes no mercado. Assim, os pacientes tinham apenas a opção do produto de marca para realizar seus tratamentos.
Dois novos genéricos, ainda inéditos no mercado, ganharam registro da Anvisa e vão dar mais alternativas para médicos e pacientes. Os produtos são a Nitazoxanida e o Cloridrato de Mebeverina. Os dois medicamentos são de substâncias que ainda não tinham concorrentes no mercado. Assim, os pacientes tinham apenas a opção do produto de marca para realizar seus tratamentos.
A Nitazoxanida é utilizada no tratamento de infecções intestinais causadas por protozoários, helmintos e também para gastroenterites virais provocadas por rotavírus e norovírus. O Cloridrato de Mebeverina é indicado no tratamento sintomático da dor e de espasmos abdominais, distúrbios intestinais e desconforto intestinal relacionados à Síndrome do Intestino Irritável.
Com a aprovação, os pacientes e médicos terão novas alternativas de tratamento a um custo menor. Isso porque, ao entrarem no mercado, os genéricos têm um preço pelo menos 35% inferior ao valor de tabela do produto de referência.
A Anvisa tem dado prioridade ao registro de genéricos inéditos, pois o registro de um produto deste tipo significa um aumento real na oferta de opções e na queda de preços provocada pela concorrência de mercado.

Nova vacina contra gripe chega ao mercado ( O Globo )

16/12/2014 - A primeira vacina contra quatro cepas do vírus influenza, da gripe, acaba de ser aprovada pela Agência Nacional de Vigilância Sanitária (Anvisa) — dois tipos da A e dois da B, aumentando as chances de proteção contra a doença. A formulação tetravalente estará disponível somente na rede privada, a partir do começo do ano que vem. Indicada para pessoas com pelo menos 3 anos, a novidade, internacionalmente conhecida como Fluarix Tetra, tem como principal vantagem a inclusão de mais um tipo de cepa do vírus influenza B.
— O impacto da influenza B é um pouco menor do que o da influenza A, já que circula em menor intensidade, por isso também que não se fala muito dela. Mas em 2013, aqui no Brasil, foi registrada uma importante taxa de internação pelo vírus, que respondeu por um em cada quatro casos de gripe — afirma a pesquisadora da Universidade Federal de São Paulo (Unifesp) Nancy Bellei, acrescentando que a faixa etária mais acometida é a das crianças e dos adolescentes.
No país, a vacina até então oferecida pelas clínicas e pelo governo é a trivalente, com proteção contra três cepas do vírus — dois da influenza A e um da B. De acordo com o Ministério da Saúde, a imunização atual pode reduzir em até 45% o número de hospitalizações por pneumonias e 75% a mortalidade por complicações do vírus influenza. Ainda não há previsão para que a nova vacina entre no Calendário de Vacinação.
EFEITO COLATERAL É MITO
Segundo Bellei, não passa de um mito a ideia de que a vacina contra gripe poderia causar a doença. Ela esclarece que há cerca de 182 vírus respiratórios em circulação, dos quais apenas quatro são influenza. Por isso, diz, é muito comum as pessoas confundirem um simples resfriado com uma gripe.
— Para ser gripe, é preciso estar há três dias com febre alta, ter dores na juntas, tosse. A gripe é persistente, diferentemente do resfriado — diz Bellei.
A doença, como alerta a Sociedade Brasileira de Imunizações (SBIm), pode afastar um indivíduo por até cinco dias da escola ou do trabalho. Os mais suscetíveis a graves complicações são crianças, idosos, mulheres grávidas, pacientes com doenças crônicas e profissionais da saúde.
Essas pessoas, que compõem o grupo de risco, são as que têm maior indicação para a vacina. Bellei lembra que ela deve ser repetida anualmente, devido à queda do nível de anticorpos e à mudança na composição, atualizada periodicamente de acordo com as orientações da Organização Mundial de Saúde (OMS).
